Our Heart Failure Center is managed by Jason Astrin, PA-C, MBA, DFAAPA, a certified physician assistant in practice since 1994 with a special interest in heart failure management.
At the Heart Failure Center at Good Shepherd, we focus on helping Heart Failure patients proactively manage their heart disease and improve their quality of life.
Our goal is to reduce hospitalizations and improve heart failure patient care by providing intensive personalized therapy and by developing individual programs that educate patients about heart failure, optimize chronic medications and help patients manage their disease.
All of this is accomplished working side by side the patient’s primary care provider to ensure seamless treatment of the patient’s heart disease.
Components of the Heart Failure Center at Good Shepherd:
* Physician Assistant directed care with collaboration from board certified invasive and interventional cardiologists
* Intensive, comprehensive patient and family caregiver education about heart failure with an emphasis on a low-salt diet, medications, signs & symptoms that signal worsening heart failure, daily weights and management strategies for problems
* Vigilant, frequent follow-up with hospital discharges
* Optimization of medical therapy (ensuring patients are prescribed the appropriate medications in appropriate doses) with published guidelines based on large-scale randomized, controlled clinical trials
* Increased access to healthcare professionals for problems by telephone or “walk-in” appointments
* Early attention to signs and symptoms of fluid overload
* Supplementation of in-hospital education with outpatient education
* Attention to behavioral strategies to increase compliance
* Emphasis on addressing personal, financial and social barriers to compliance
* Assessment and assistance in management of social and financial concerns
