Medical Records
To obtain a copy of your medical records from Good Shepherd Medical Center in Longview, TX, a completed and signed Release of Confidential Information Consent form must be submitted to our Medical Records department in person with a photo ID. Our Medical Records department is located on the first floor of the main hospital down the hallway leading toward the Red Apple Elevator.
