Birth Certificate information
Good Shepherd Medical Center’s Health Information Management Staff will assist with completion of your baby’s birth certificate at no charge. The Health Information Management Staff will come to your room on the morning following your delivery to complete the birth certificate. Please complete the birth certificate worksheet prior to this visit. If you do not want the hospital’s assistance with the birth certificate, the process can be completed at the Gregg County Courthouse for $30.
