Quality Care for our Patients

The process of care measures is the standard of care for the diagnoses identified as Core Measures, which include diagnoses such as Acute Myocardial Infarction (Heart Attack), Pneumonia, Surgical Care, Children’s Asthma, Stroke, Heart Failure and more.

The process of care measures is a national set of standardized care expectations for specific diagnoses, which have been studied and proven to promote maximum wellness and improve patient outcomes. Not all of the care measures apply to each patient but if they are found to be appropriate for the patient, they should be implemented. This process requires the patient, nurse and physician involvement to ensure the patient receives the best quality of care available for their particular diagnosis.

The Process of Care Measures percentages below were collected between April 2010 and March 2011 by chart review and submitted to regulatory agencies.
